BROCKVILLE POLICE AND STINGERS SUMMER PROGRAM REQUIRED FORMS

All youth who would like to participate in the Summer Program must complete the Youth Participant Registration Form and have a Parent or Guardian sign the Stingers Liability Waiver. Youth who will be using the free bus system to come in from different communities are also required to have a signed permission form.
